IMMUNIZATIONS POLICY *****
Beginning June 4, 2012
The
Health Department will be taking appointments to administer your child’s
immunizations
as well as continuing to provide the other
regularly scheduled clinics throughout the county.
The
appointments will be located at the Daviess County Health Department Office

Your child may receive immunizations by
appointment or at a clinic if:
Uninsured: NO health insurance
Underinsured: Health insurance does not
cover vaccines
Medicaid: MUST BRING MEDICAID CARD
If You Have Health Insurance
Coverage, Please Contact Your Primary Care Provider
(or you may contact one of the
providers from the link below)
When receiving immunizations from the
Daviess County Health Department
You will be charged a fee of $8.00 per
injection
